We are pleased to bring you the my mhealth app, offering free access to support residents living with diabetes, asthma, COPD or cardiovascular disease.

Your GP Practice, as part of the local healthcare system, is offering free access to my mhealth, providing some extra support to help you stay well and feel more in control of your health and long-term condition.

My mhealth explains your condition in simple terms and gives you tips and tools to help you feel more confident day to day. It doesn't replace your usual care, your GP practice team are still there for you, just as before. It works alongside them, to help you understand and manage your condition yourself where you can.

You may receive an invitation from your GP practice to register a free account for my mhealth. You have been offered this because your GP practice has reviewed your medical records and noted you have a long-term condition that would benefit from access to my mhealth.
